We accept all dental insurance plans that allow patients to choose their own dentist. All patient deductibles and percentages are due at the time of service.

As a courtesy to our patients, the office will submit claims to one insurance company. We will make every effort to work with your insurance carrier to maximize the benefits available to you through your particular policy.

Please realize that professional services are rendered to you the patient - NOT the insurance company. The patient is responsible to the dental office for payment and is responsible for ALL moneys that the insurance company does not pay.

Remember that insurance only pays a portion of your dental investments. To avoid disappointment, make sure that your dental insurance assumptions are correct.

Please know the following information about your dental insurance plan:
The effective date of your coverage, your benefits, any waiting periods before certain procedures can be performed, any exclusions or limitations to your coverage.

Each insurance patient should bring the following to their first visit:

Insurance card - the card should include the following information:
Dental Company's name, address and phone number to confirm benefits.

Insured's information:
Insured's name, address, phone number and birth date.

Insured's employer information:
Name, address and phone number.
